Membership Information

Membership Eligibility
In order to be eligible for membership, applicants must have a primary diagnosis of a serious and persistent mental illness (SPMI) OR a serious mental illness (SMI); AND be 18 years or older; AND not pose a significant or immediate threat to the general safety of the Clubhouse community; AND has demonstrated safe behavior in the community without supervision for a period of time; AND are expected to be independent (in mobility and self-cares) and able to be unsupervised, as staff cannot monitor members at all times.


Membership Process

Applicants will complete the Lighthouse on Marshall Application, a release of information form for their county case management OR other Mental Health provider, and will have that worker complete the Membership Support and SPMI/SMI Verification form & Activity Levels form. If an applicant does not currently have services provided through county case management, they can work with another designated provider such as a therapist or psychiatric provider to complete these requirements.
